Forms and Applications

Below is a list of all DC Office of Personnel forms that are currently available online. They are available in one of two formats, as an Adobe Acrobat Portable Document Format (PDF) file or as a Microsoft Word form.

Forms marked with an asterisk (*) can be viewed with Adobe Acrobat Reader. To print a form, click the printer icon directly above the PDF document. These forms must be filled in by hand and returned by mail or fax to the DC Office of Personnel. For more information about Adobe Acrobat Reader, please see the footnote below.

Forms marked with two asterisks (**) are in Microsoft Word. Most information on these forms can be entered from your computer. However, they still must be printed so they can be signed and certain information can be completed. They must be returned by mail or fax to the DC Office of Personnel.
